1 ) Is my personal information
safe?
Yes. All data is secured through 128bit SSL encryption. Your personal
information is held on a separate secure database server that is not accessible
outside the ClearChoiceConsults domain. Our servers have firewalls and flawless
redundancy so that your personal patient files which contain the questionnaire,
personal information, medical history/records, and payment method remains
secure and confidential. We monitor all access to the site and web logs for suspicious
activity. In addition to the privacy protections that we provide, our employees
and business partners are independently responsible for ensuring compliance
with this Privacy Policy.
2 ) How much is the consult, and what do I get for the consultation fee?
The price of the consultation is $150.00. ClearChoiceConsults establishes your medical records along with all pertinent
documentation regarding your medical condition(s). ClearChoiceConsults then
arranges for a private telephone consultation with one of our experienced
physician addressing your specific medical concerns and needs. If the physician
determines that prescription medication(s) is warranted, the prescription(s)
will be hand written and shipped out.
3 ) What type of Medical Records are required?
In order for us to maintain legal guidelines, you must fax evidence of your
existing medical condition on your doctor's letterhead, prior to your
consultation. The doctors require documents that substantiate the condition(s)
for which you are requesting medication. Medical records must be less than 1
year old. Please fax your medical records to our fax number: (888) 259-6502.
You must also fax a copy of your Photo ID. Please insure your fax is clearly
marked with your name and number of pages. We use an electronic capture for
faxed medical records, and only the doctor can access and view your medical
records.
4 ) What is the cost of the prescription medication?
While we are offering the Direct Prescription program the medication is filled by you at your local pharmacy. You may use your insurance for this to save money.
5 ) How do I pay for my consult/prescription?
All payments will be made via COD and paid for at the door upon delivery of
your package. Please make the Money Order payable to ClearChoiceConsults. This will
ensure complete discretion of the contents of your package.
6 ) Do you accept payment by Credit Card?
No. The only method of payment is COD, this is for your protection as you will
only pay AFTER you have received your prescription.
7 ) How long does the process take?
After creating your account you will receive your consult the same day. Upon
completion of your consultation your prescription will be hand signed by the
doctor and express mailed the next day to the pharmacy.The pharmacy will
receive your hand signed prescription the following day and fill it. You will
then receive your order the next day. From start to finish the process takes
approx. 72 hours. This is longer than the previously understood time of 24
hours, but as the industry has changed so has the shipping times.
8 ) Do I need medical records?
The doctor will require some form of proof of your condition. He will need to
see that you have an ongoing need for the medication he is prescribing. By
providing these records to us we can continue to legally provide you with
service for as long as you need it. Without these records we will not be able
to provide service to you.
9 ) How are the orders shipped?
All packages are sent in envelopes via FEDEX.
10 ) Are your Doctors board
certified?
Yes, all our doctors are licensed to practice medicine in the USA and are based
in the USA.
11 ) What if the doctor does not prescribe medication?
If the doctor finds that medication
is not needed in your case you haven't lost a thing since we do not accept
credit card payments.
12 ) What state do you ship to?
We ship to almost every state, with the exception of: KY, TN, VA, and WV.You can view a complete list of the states that we ship to through the "Join Now" section of the websites, by clicking on the state drop down box.
13 ) How do I track my delivery?
You will be sent your tracking number along with your COD total through the
email address you have provided us during the sign up process.
14 ) What is the minimum age?
You need to be at least 21 years of age to order any medication,
consultation or prescriptions from this site. This is one of the reasons why we
require your photo ID to be sent in along with your medical records.
15 ) Can I return my medication for a refund?
No. Federal laws prohibit the return of medication.
16 ) What are your operating hours?
Standard business hours are Monday through Friday 9am - 5pm EST. If you
need to reach us outside of these standard business hours please send us an
email at support@ClearChoiceConsults.com. When the office reopens on Monday
morning, one of our customer support staff members will respond to your email.
17 ) Can I change my shipping method, after an order has been placed?
Yes. To do so, please contact customer service via email with your name and desired shipping method.
18 ) How do I request a Reconsult?
After you have received your order, 25 days later you are eligible to request a Reconsult. To do so you must login to your online account you established with us at the time of registration. Then click on "Create Medical Questionnaire." This will then forward your request for a Reconsult to us and we will see that you are scheduled and consulted promptly.
